Training Program Forms

Training programs for First Responder, EMT-Basic, EMT-Intermediate or EMT-Paramedic must be approved by the EMS Regulatory Board.

Applications must include a $100 payment, payable to the "State of Minnesota." After the EMSRB receives a new training program application, a site visit is conducted and formal paperwork is begun.
