NO CPR FORM

The "No CPR" form is available for use by individuals and their attending physicians when they wish to leave instructions for emergency personnel not to perform cardiopulmonary resuscitation. The "No CPR" form is a simple & clear alternative to a "Do Not Resuscitate" (DNR) form, and has been reviewed and approved by the Medical Direction Standing Advisory Committee for use in the pre-hospital setting. Each ambulance service's medical director determines the protocols for responding to DNR or No CPR forms.
